Dysfunctional Thought Record

This table is particularly supportive for people who struggle with harmful feelings and need to figure out this negative. Learning about what provokes certain voluntary thoughts makes them easier to tackle and modify them. The DTR Worksheet is divided into seven columns:

1. On the left side of the column, there is section to note down Date and Time a dysfunctional thought arise.

2. In the second column write about the Situation. The user is instructed to illustrate the event that lead up the dysfunctional thought in detail.

3. The third column is for the Automatic Thought. This is where the dysfunctional automatic thought is recorded.

4. The fourth column is for the Emotion or emotions elicited by this thought are listed.

5. The fifth column is label as Cognitive Distortion. This column is where the client will recognize which cognitive distortion.

6. The six column is for the client to write down Alternative Thoughts that are helpful and useful to replace the pessimistic one.

7. The last and 7 column is for the client to mark down the conclusion of the exercise.
