Changing Patient-Centric Care with Machine Intelligence
In 2014, The Milken Institute published the study Chronic Disease and Wellness in America. They found that “In the realm of health, there are no more deadly and costly threats to patients, their families, and the economy than chronic disease.”
By 2025, the RAND Corporation project that chronic diseases will affect an estimated 164 million Americans — nearly half (49%) of the population.
The Center for Disease Control and Prevention estimates that Chronic diseases account for more than 75% of health spending in the U.S.
Guidelines for the prevention, care, and treatment of every Chronic disease emphasize the importance of self-management by persons who suffer from chronic diseases.
Type 1 diabetes is a Chronic Disease that can develop at any age (most often in children, adolescents and young adults). There is no known way to prevent the onset, where the body does not produce enough insulin and where self-management is essential to a person’s health. Intermountain Healthcare has developed a program to support persons who have Type 1 Diabetes and has identified the distinct support needs of the various age ranges of persons who have Type 1 Diabetes.
Intermountain Healthcare and CognitiveScale have been collaborating to enable young adults ages 18 to 25 who have Type 1 Diabetes to manage their Blood Glucose Levels more effectively. This ambitious project uses the protocols developed by Intermountain Healthcare and the application developed by CognitiveScale to engage the young adults by delivering personalized information and insights produced by machine learning algorithms that help them manage their activity, diet, and sleep patterns to support their efforts to regulate their blood glucose levels within desired ranges.
Our ENGAGE Health care concierge is powered by CognitiveScale’s cognitive healthcare cloud and the application privately and securely analyzes personalized information that is delivered to the individual patient.
Our goal is to provide continuous service, support, and information delivered through mobile devices to the young adults who are working diligently to manage their disease and optimize their health.
