How to control my oily scalp problem that gets compounded with dust action. Please help?

D Venkatraman asks: Sir, I am in an advanced stage of baldness that almost covers 80% of my scalp. In such a scenario I am worried about where the required hair grafts would come from. How many follicular units/hair grafts would the front, middle and top regions of my scalp require? Can my beard hair be used as hair grafts? How many sessions do I require to attend for a hair transplant? Please explain.

In his response, Dr John Watts informs Venkatraman that in an advanced stage of baldness, a proper hair transplant with dense hair becomes difficult if there is no possibility of meeting the required donor hair for a transplant.

“In such a scenario, the donor hair can come from the limited stock of donor hair grafts at the back of the scalp but they would be inadequate. Hence, we may have to look at the chest, beard and hair grafts from the pubic regions. Still, it won’t suffice,” explained Dr John Watts.

The estimated hair grafts that Venkatraman may require could be in the range of 7000 to 9000 hair grafts/follicular units. “This includes about 3000 on the front of the scalp, about 2000 follicular units on the top and another 2000–3000 grafts on the crown region. This means that the available donor hair would not be enough,” explained Dr John.

Furthermore, the growth capacity of transplanted hair differs from one region to another and that becomes another factor in the final results.

Alternative Solutions:

In light of the above facts, Dr John Watts advised Venkatraman to try alternative solutions such as wearing a hair patch or a wig. The other alternative is to opt for a closed shave look and go for Scalp-Micropigmentation (SMP) which gives a natural look. “If your goal is to have natural hair that does not look very dense then you can opt for a hair transplant procedure. You will still have decent hair to show off,” advised Dr John.

Ricky asks: Sir, I ride a bike every day wearing a cap to avoid dust. Yet, I am not able to control my oily scalp problem that gets compounded with dust action. I take a head bath twice a day. However, I am struggling with dandruff. I also use Minoxidil. Please explain what I should do.

In his response, Dr John Watts informs Ricky that some people have oily scalp problems and, in such conditions, it is not unusual to take a head bath twice a day. If dandruff is an issue, one may take anti-dandruff medication.

However, Dr John advised against using Minoxidil until and unless the oily scalp problem is resolved first. “One home remedy you can use for your oily scalp problem. Take lemon juice and apply it to your scalp once a week. Keep it for 15–20 minutes and rinse it off with water. If the problem persists, consult a dermatologist,” he advised.
