At the heart, I’d say trust & safety (tightly coupled to communication issues — the worse the communication, the less trust & safety, the less trust & safety, the worse the communication).

Scale can start the decline as this introduces new personalities without the pass through the fire experience you get with very small teams. Failure to actively manage culture means that it develops organically (i.e. in a Darwinian manner — not something conducive to friendly collaboration).
