An Open Letter to Congress from Healthcare Providers-in-Training and Their Allies at NYU School of Medicine

This is an open letter to Congress from healthcare providers-in-training and their allies at NYU School of Medicine. This letter reflects the personal views of the signatories and is in keeping with the patient-centered NYULMC mission statement.

We, the undersigned medical students, residents, and allies at NYU School of Medicine and Bellevue Hospital, recognize the need for healthcare reform in the United States. However, we are alarmed at the state of political discourse surrounding this issue, particularly with regard to the recent efforts to repeal and replace the Affordable Care Act. Lawmaking behind closed doors has no place in the people’s government and is especially concerning when the legislation in question is a life-or-death issue for millions of Americans.

While there are many ongoing issues with the ACA, the secrecy and haste surrounding the attempts to repeal and replace it makes us seriously question the motivation of the politicians responsible. This haste and secrecy are as concerning as they are puzzling. Are our leaders more concerned with scoring political points than with saving lives?

Politicians in Washington are far removed from the consequences of the current issues in our healthcare system. As physicians-in-training, we see how these issues play out in people’s lives every day. We cannot stay silent and watch our patients suffer as politicians reduce the vital issue of healthcare reform to a game of partisan brinksmanship.

The ACA is not a perfect solution. Many who gained health insurance under this legislation continue to lack truly affordable access to healthcare. Healthcare in the United States is extremely costly compared to other industrialized nations, and we understand that the ACA did not fix this problem. However, this major economic issue does not represent a life or death situation in the way that access to healthcare does. The ACA was drafted with the goal of coverage and quality of care for all Americans, and does represent a significant step toward real access to affordable healthcare for everyone.

The exorbitant cost of healthcare in the US is a longstanding problem that has its roots not in the ACA, but rather in the for-profit focus of the American healthcare system, which is inherently structured to prioritize financial profit first, and science, health, and efficiency after. Unfortunately, extreme partisanship has prevented lawmakers from acknowledging this truth and the gains made under the ACA, thus hindering any meaningful improvements to the bill and realistic tackling of the cost conundrum. The recent pattern of intentional sabotage of the successes of the ACA in order to score political points is grossly counterproductive and morally reprehensible.

The secrecy surrounding the Republican replacements, the American Health Care Act (AHCA) in the House and the Better Care Reconciliation Act (BCRA) in the Senate, the rush to push toward a vote, and the refusal to listen to the condemnation of many healthcare organizations, is further evidence that, for politicians, healthcare has become a partisan game rather than a human rights issue. Furthermore, the vote was pushed despite the CBO determination that the bill would take health insurance away from 22 million Americans over the next decade, result in an immediate rise in premiums, particularly for those in their fifties and sixties, and shift federal funding from the poor to the rich.

Given the seriousness of healthcare reform and the impact that it has on the everyday lives of all Americans, we demand that the discussions and decisions surrounding this issue be treated with the gravity and deep consideration that such matters deserve. We are outraged by the irresponsible and unethical decision to force the AHCA through the House, as well as the secrecy surrounding the BCRA, which was pushed toward a rushed vote in the Senate for a second time after minor revisions that did not adequately address the major flaws with the bill and without waiting for an updated CBO score. Following its apparent failure, there has been a push toward a “repeal now and replace later” measure that would also have devastating consequences for the American people. The CBO estimates that a clean repeal of the ACA would leave 32 million uninsured and would cause premiums to double over the next decade.

Therefore, we urge Congress to finally stop the attempts to dismantle the ACA and instead have a thoughtful, thorough, and informed discussion on healthcare reform. Only then can meaningful improvements to the ACA be proposed.

As current and aspiring physicians, we do not always immediately know the solution to a problem or what the best treatment is for a patient. However, we know how to approach a problem in a thoughtful and thorough manner that helps us care for and give our patients the best possible outcome. We urge our elected officials to approach the problem of healthcare reform with the same spirit and motivation that we as healthcare providers have in caring for our patients. The current partisan approach to healthcare is extremely disheartening and alienating for us as physicians-in-training, and it severely limits our ability to provide the best treatment for all of our patients.

In that spirit, we hereby present some ideas about what a discussion on healthcare reform legislation should look like:

1. Protect patients with pre-existing conditions in every state from all discrimination and recognize that this can only be achieved by mandating buy-in from healthy individuals to offset the costs.

2. Require insurers to cover preventative services and essential health benefits, including, but not limited to, pregnancy, mental health, and addiction coverage.

3. Offer healthcare plans that are not only affordable, but are also realistically accessible to all Americans. This includes, but is not limited to, Medicaid expansion, as evidenced by its success in multiple states, including Ohio, Kentucky, and California. Include regulations to prevent monopolies by one or a few dominant companies, thus facilitating competition and ensuring sufficient health insurance options for all Americans.

4. Use evidence-based input from expert witnesses, patients, and open hearings in Congress. Include objective and thorough study of the successes and failures of existing national and international community health systems and social safety net models, such as HHC. This includes consideration of regulations that keep healthcare costs down, such as government negotiation of drug prices, mandatory transparency in medical procedure costs, and government-sponsored insurance plan options.

5. Ensure that expert witnesses, patients, and those responsible for drafting legislation include equitable representation of women and underrepresented minorities. In drafting healthcare legislation, it is important to bring diverse perspectives, particularly when women and underrepresented minorities face the greatest barriers to healthcare access and implementation. As physicians-in-training, we have found that we learn best from our peers and patients with diverse perspectives and backgrounds, and healthcare affects everyone.

As healthcare providers, we are trained to treat the patient, not the disease. Politicians need to follow this principle and draft healthcare legislation that focuses on the American people, not on profit and premiums.
