Those are definitely points that deserve to be on the list! Particularly the lack of an empowered business person working with the team is something I’ve seen having a rather devastating impact. Such teams either end up building to a fixed specification or their (or an unempowered product owner’s) decisions get overruled late in the game, resulting in expensive rework.
