Sometimes, most of the website owners forgot to edit and give every page and post of their website a unique meta description to avoid duplicate meta description. If you have a duplicate description, search engines might think you have a duplicate content in your web pages and thus it will affect your ranking.
Bad Meta Description this kind of meta description is not optimised for SERP display. Meta description needs to be 120–160 characters and contains keyword so that people would think that your site might have the answer they are looking for, thus your site will get high CTR in SERP for having compelling meta description.

Alt attributes make it possible to enter an alternative description in the HTML code for every image on a website. This description appears if, for some reason, the image cannot be displayed. Search engines ability to recognize the contents of images is still limited which is why they rely on the alternative description in order to determine what the image contains. The alt attribute can influence the content of which search engines consider a website to be relevant. Ideally, the alternative descriptions should also contain the search terms for which the website is optimized.

I know what you are talking, keyword density is an old SEO method but still we need to check how many keyword you mentiond in the ration of total number of words you have in the particular post/page. Because high keyword density is a Keyword Stuffing which will get you penalized and thus will down your rankings of even out of top 100 organic search results.
