Open Letter of Support for Race-Conscious Approaches to Promote Health Equity

Health inequities among racial groups have been present since well before W.E.B. Dubois described the relationship between race and life expectancy in his seminal work, The Philadelphia Negro (1899). A growing body of literature demonstrates that race and ethnicity, as proxies for racism rather than biological difference, are significant predictors of healthcare quality, even after controlling for socioeconomic conditions. Health inequities have historical underpinnings and are also steeped in ongoing interpersonal and structural racism in health care. Despite knowing about these inequities for decades, Black and Latinx patients continue to receive worse treatment, even in our nation’s premier hospitals.

The interventions most commonly employed in medicine to address health inequities, such as implicit bias training, diversity and inclusion efforts, and evidence-based practices, although important, have not resulted in a meaningful reduction in health inequities. Drs. Michelle Morse and Bram Wispelwey have bravely argued that given the historical and ongoing role of racism in perpetuating these inequities, new race-conscious (i.e. not colorblind) policies are needed. They laid out a well-reasoned and practical framework–the Healing ARC–through which racism can be named and systematically redressed to promote restorative justice and equitable outcomes for historically harmed populations.

As professional colleagues, we condemn neo-Nazi attacks on Drs. Morse and Wispelwey, as well as their colleagues fighting racial inequities in other fields. We wish to express our solidarity with Drs. Morse and Wispelwey and all others who are exploring innovative, race-conscious ways to address inequities in health care rooted in racism.

We further call upon health care institutions and universities to protect and support innovative thinkers like Drs. Morse and Wispelwey who seek new ways to promote healthcare equity. And most importantly, we call on health care institutions, organizations, universities, and workers to take strong, public positions in support of race-conscious interventions to address racism as an ongoing public health crisis. The best response to neo-Nazi threats is exponential growth in programs like the Healing ARC across the country.
