Course Overview: The contributions of pharmacists to the clinical and community care team in areas such as: collaborative drug therapy management; health information technology and personalized medicine are essential toward improving the quality of care, cost-effectiveness, patient experience and clinical outcomes.

Learning Objectives:

  1. Explore the role and implementation of the pharmacist in the PCMH;
  2. Explore new care models involving the integration of community health workers (CHWs) and pharmacists;
  3. Discuss the expanding role of the pharmacist in diabetes self-management education (DSME) and diabetes prevention programs (DPP); and
  4. Discuss the value of comprehensive medication management in the community and clinical care setting.
