I really enjoyed reading this, and the challenges are familiar. A good Agile introduction gets people excited about the mechanics of the new roles and meetings, but the purpose and context of them is crucial.

One test I might add is around continuous improvement. Something along the lines of: “Does the team regularly review how they’re working, and do they have the opportunity and authority to make improvements?” Without this, the team will not be able to adapt to changes in their environment, and their output will become weaker.

Another suggestion is similar to Hafiz’s. “Does the team have a clear definition and measurement of their output?” Without this visibility, it is hard to plan, review and improve.
