Amazingly, no one has yet assembled a complete environment in which two or more people can easily discuss, review, and approve changes to a document through an interface that doesn’t get in the way.

Actually, Editorially solved this problem really well for the year or so it was around. But the user base was not big enough to satisfy investors or justify costs, and it shut down. What people need is not the same as what is a viable business.
