I’m from the old school of corporate software development and a senior developer at Microsoft, and one key principle that may have helped here is Developers are not good Testers. They have different mentalities. A developer is a builder and has the perspective of making it work. A tester is just the opposite, he is a breaker and looks at everything to tear it down. these 2 roles are necessary and complimentary on a software development team. A good tester is not a junior programmer, but is a skilled craftsman on his/her own career path. Code reviews are good for design and implementation reviews, but not for testing. I truly appreciate from experience that bugs are a normal part of software development. Bugs can not be avoided, only managed. Microsoft software release candidates per always based on bug management thresholds.
