Open ended questions are great, but I would be wary of negative priming/bias towards when asking about challenges specifically.

To get at potential problems they would want addressed, we ask the ‘three wishes’ question. This is usually after they have explained their process the last time they did something (not how they might in the future). It can get at the real challenges without biasing them towards finding issues.

Don’t let them off the hook if they only give you one or two wishes either! The third one can be very helpful once they need to wrack their brain.
