This Privacy Policy (“Policy”) explains how we at Cronic Care DE LLC, Remote Patient Monitoring and Virtual Care, (collectively, “Company”) handle the information we collect from or about you when you use our website (www.cronic.care).

If you are a Cronic Care member, any information collected by Cronic Care healthcare providers will be subject to the Notice of Privacy Practices of your healthcare provider. Any information collected through your Cronic Care -powered device will be subject to the privacy policies in place by the device manufacturer. Device data received by Cronic Care will be subject to the Notice of Privacy Practices of your healthcare provider.

To exercise the rights described above, you or your authorized agent, must send us a request that:
