I strongly agree with all the points except the one about placeholders. Our research has proven, that if the font color of the placeholder is distinguishable from the input color, a placeholder is actually improving the UX.

Also, the multinational companies would have trouble sticking to this format of phone number entry, unless the require the country first, and only then display the phone field. This will make the UX more confusing.
