Thank you Dave! This indeed is an improvement. More degradation proof thanks to the specific questions and the direction of the process.
And I agree it needs a link to motivations and actions that the thoughts and feelings of the person could evoke. So I agree with earlier comments that the pain — gain part has value on the canvas.
My first idea of it would be to put thinking high, feel lower and pain at the left (aspiring to move away form it and leave it behind and gain at the right as the attractor). Inside the head that is.
Listing the pains and gains could also shed a light on wether the person is fear motivated of inspired by possibilities.

At this early point — just read it– I doubt of wonder wether simply making it a four square would suffice.
