Monthly expenses fluctuate depending on various factors, including deductible status, usage patterns, and insurance coverage types. élos Healthcare’s services are supervised by your physician, and your clinic bills your insurance directly for remote patient monitoring services. If you’re contemplating enrollment with élos Healthcare and have inquiries regarding potential costs, it’s advisable to contact your insurance company directly and provide the billing codes provided below for an estimate.

During your next appointment, inquire with your physician about whether élos Healthcare is suitable for your needs, and reach out to one of our enrollers to begin the process.

How can I get in touch with élos Healthcare? And how often should I expect to communicate?
élos Healthcare will arrange regular telephone appointments with you, tailored to your specific needs. If you’re feeling unwell, you can always contact the Sage program team with any queries. Additionally, if we notice any unusual vital signs, we’ll reach out to ensure everything is okay.

Should I continue communicating with my primary care physician?
Absolutely. élos Healthcare serves as an extension of your primary care team. You’ll continue to see your primary care physician as usual, and all vital information from élos Healthcare will be accessible to your physician during your visits.

What’s the process for taking my vitals and how frequently should I do it?
élos Healthcare will furnish you with devices that directly communicate with the Sage team. These devices function similarly to others you may already be accustomed to. It’s recommended to take your vitals each morning before consuming food, beverages, or medications.

Is élos Healthcare covered by insurance?
Remote patient monitoring and Chronic Care Management are covered benefits under Medicare. Patients may be responsible for a monthly copayment or coinsurance, depending on their insurance plan. Our Sage Team can provide assistance in understanding the potential cost during your meeting with them.
