49A is the application you must fill out to get the card in case of loss of PAN in any case. So follow these steps to get your new duplicate PAN card from the original.

First go to the Request Form 49A online for PAN card reissue

Enter your PAN card number first

Choose an option as your “I lost PAN card / Damage and only that reedited”

Select the copy of PAN card / copy of the letter of award / No document / copy FIR

Give personal information such as PAN card

Give correct mobile number and other details

Send the application form stating

Now if all details required (*) fill that application shall be submitted

A click confirms and gets the acknowledgment copy print
