“Accountable safety: if two conflicting hashes get finalized, then it must be provably true that at least 1/3 of validators violated some slashing condition.” I suppose this is clarified in the next paragraph but I gather the “accountable” part means “…, and also provable which validators made up that 1/3.”

“But this introduces a safety flaw: two different hashes can get finalized in different epochs!” Here “conflicting” seems clearer than “different”. No one expects the same hash to get finalized in every epoch…
