Just as you wouldn’t blame a knife for being used incorrectly, you can not blame RDMBs for the same. Relational databases are tools. They are not universal problem solvers, they do not think and they do not make their own decisions.
With every tool it is up to the individual(s) to implement it correctly and safely.

RDMBs are barely the reason why:

enterprise systems fail so regularly

Relational databases are hardly the ‘poison’ that you mention. Users are.
