What you suggest will require a level of discipline not likely to appear any time soon. It will also require a much different set of incentives. How do we craft those incentives when there are no incentives for change? That old sense of responsibility to the greater good held by leadership has been replaced by a disregard for that greater good.

Not sure what the answers are but since we are all complicit in the creation and nurturing of these problems we should be able to come up with some reasonable solutions. The first step is a broad recognition that we have a problem.
