If we are to have insurance for everyone, we need to subsidize it for those who have low incomes — and this has nothing to do with whether they “deserve” help, or even with whether healthcare is a right. It’s about spreading the cost of a universal human risk as universally across the humans as possible.

Frankly, the feds need to stay out of the idea of becoming insurances companies. Rather they need to implement a policy that every insurance company must offer income based policies some of which must be free to those who qualify.
