The designers do the hard work to make it intuitive or the users do the hard work to figure it out.

Bingo. Training should be done to show users how to operate software if good UX can’t make it apparent.

Very stupid example: you want to save a document. I could put a save button on the toolbar, put it within File menu, or bury it under File -> Document Actions -> Write To Disk.

If EHR does the latter and ppl need training how to save their work, u will almost always need useless training and hand holding, where a better UX design would have avoided this.
