According to the American Heart Association, more than 100 million Americans are afflicted with high blood pressure. That’s nearly half of all American adults. So, it’s pretty safe to say this is a huge issue facing American public health.

Interestingly, the disease disproportionately affects African-Americans, Asian-Americans, and Native Americans, compared to their White American counterparts in the same weight category and similar neighborhood education level, according to a study by Kaiser Permanente (46.0% for African-Americans vs. 34.9% for White Americans).

What Makes Transfer Doc Unique?

Transfer Doc takes a unique, three-fold approach to effectively managing high blood pressure:

  1. A patient-facing application for tracking items like blood pressure cuff readings, weight, medications and side effects, and progress towards personal health goals.
  2. A clinician-facing dashboard for reviewing patient data and assigning patient tasks, with tools including clinical decision trees to assist in diagnosis and modifications to treatment plans at the point of care.
  3. Using anonymized clinical data to power machine learning algorithms, enabling predictive analytics and huge boosts to a health system’s population health capabilities
