The “trans” part of transgender has nothing to do with appearance. A trans person has always felt like they are not the gender they were assigned at birth. This feeling, this identity, causes them a lot of pain in the way of dysphoria, where they feel like their body isn’t theirs, because it doesn’t reflect who they are inside. *So they change their body to try and reflect their identity, not the other way around.*
