The US has, and had, a two-man rule. You’ might notice that whenever a pilot goes to the bathroom, a flight attendant takes their place in the cockpit. As a result, there is always a second person in the cockpit who can open the door from the inside. Of course, the pilot who remains could try to overpower this flight attendant, but that’s a much tougher thing to do when the flight attendant just has to get the door open before the pilot has a chance to overpower them.
