This true, but also pretty much human nature. It’s rare for any person or institution to acknowledge a mistake in an honest or actionable way.

One thing I’ve always admired in military procedure is the after action report. An honest look at what happened, why, and how to improve the next time is invaluable. In theory this is part of what an ombudsman is supposed to do for a media org, and it’s a position I think even non-media companies could benefit from.
