What Is Laparoscopic Surgery?

Laparoscopic surgery is an advance surgical technique in the field of medicine. This word laparoscopy has a Greek origin. Causes-

· Ectopic pregnancy

It’s a pregnancy complication in which the fertilized egg is implanted outside the uterus. Most of the ectopic pregnancies occur in the fallopian tube. Implantation also can occur in the cervix, ovaries, or within the abdomen. The complication is tubal rupture which results in internal hemorrhage and ultimately hypovolemic shock. The risk factors for tubal pregnancies are from Chlamydia infection, PID, ovarian torsion, IUD, tubal ligation, endometriosos and exposure to Diethystilbestrol (DES) medication.

· Uterine disorders

Uterine disorders include conditions like fibroids, polyps, endometriosis, adenomyosis, benign or malignant overgrowth. Hyperplasia of cells lining the uterus (endometriosis) or outside the uterus ( adenomyosis) or any kind of over growth can result in dysmenorrhea, menorrhagia, or irregular bleeding. There could be anatomical structural anomalies like bicornuate uterus, unicornuate uterus and septate uterus which leads to infertility and other related problems.

· Ovarian disease

This includes conditions like ovarian cysts, dermoid cysts, ovarian scar, cancers and acute emergency conditions like torsion of the ovary. In ovarian disorders the hormonal imbalance occurs along with other physical manifestations like abdominal pain, discomfort, bleeding disorder etc.

· Infertility

In case of infertility diagnostic laparoscopy is the most accurate procedure for evaluating the tubal pathological abnormality and other intra abdominal cause. Less invasive diagnostic test like hysterosalpingography (HSG) and Chlamydia antibody testing values helps in the infertility work up.

· Cancer , tumours and polyps

Overgrowth of the endothelial lining of the uterus could be either benign or malignant. Benign are the non cancerous growth inside the uterus like polyp usually they are less than 1cm in diameter. Different types of cancerous growth could also result in pain, discomfort and irregular bleeding especially if the cancer is of the uterus, ovaries, cervical, vagina and endometrial tissues.

· Pelvic inflammatory Diseases (PID)

The pelvic inflammatory disease is caused by various infectious agents. It refers to the infection of the women and includes structures of the pelvis like the uterus, fallopian tubes and other related tissues. Sexually transmitted diseases are the major cause of PID. Laparoscopy is routinely used for diagnostic procedure in such cases which lead to scaring of the tissues and infertility.

Technique used in the laparoscopy:-

In this procedure small incisions (about 5–10mm) are made at the abdomen or pelvis. One of the incisions is used to insert the endoscope which is attached to a camera, making it easier for the surgeon to view inside of the joint on the monitor outside and operate. The other incisions are used to insert special tools in the abdomen to perform the surgery. It is done under general anesthesia. The instrument which is used in the laparoscopy surgery is known as laparoscope that has a high intensity camera attached to it, which gives a clear view of the inside. One of the incisions is made below the belly button. In this small opening a cannula is inserted. This is used to inflate the abdomen with CO2 gas. The number of incisions made on the abdomen depends on the disease the surgeon is attempting to confirm or rule out or treat. Also attached is a fiber optic system to halogen or xenon (cold light source), which is inserted through the port to illuminate the operative field.
