I agree Joe — it boils down to an accountability problem. And you are right to stamp it out it needs to be lead by the top.

But I also think that individuals should feel empowered to start two simple habits:

  1. The simple habit of asking who is doing what by when.
  2. Following up

I cannot imagine anyone objecting to this even if a subordinate did this to their boss.
