Design principle: Organizing information

One of the design problems I grapple with as an information architect is accessibility. I love to use spatial location on a web page, or color, to group and prioritize information. However, what’s the best way to make that organization understandable to a screen reader? I know with a visual diagram, I’d provide a separate text file describing the diagram — would you do the same thing with information grouped on a page? I haven’t seen it done.
