CREATING A DATA ENTRY FORM USING EXCEL

Data entry forms are used to collect data.

This is a step by step procedure on how I created a form to collect the data of students in University using excel web.

  1. Open your browser and go to office.com
  2. Login with your Microsoft account (Go to account.microsoft.com to create a new account if you do not have one already)
  3. Click on excel and open a new blank workbook and save it with any name of your choice

4. Go to “Insert” and create a new form

5. Excel will automatically open a new sheet in your workbook with a default table.

Open a new tab where you will create your form.
- Change the name of the form
- Add a description

6. The next step is to add your questions. Click on “Add New” to add a question.

There are different type of questions you can ask. It could be a choice question, text, rating, date or ask for a file to be uploaded.

The title of each question will automatically be added to the table in your excel workbook.

You can also choose to make the questions compulsory to answer.

My form is going to consist of 7 questions:

  • Full Name (Text)
Full Name
  • Matric Number (Text): I added a restriction to this question so it only accepts numbers.
  • Date of Birth (Date)
  • State of Origin (Text)
State of Origin
  • Gender (Choice)
Gender
  • Course of Study (Choice)
Course of Study
  • School Rating
School Rating

You can preview the form by clicking on the “Preview” button at the top.

After adding all your questions, you can change the theme.

You can access the settings of your form where you can choose who can answer, start date and end date. You can also customize a thank you message for people who fill out the form.

SHARING THE FORM

To share the form, click on Collect Responses and choose your preferred method of sharing.

Check “Responses” to see when people fill the form.

When people fill the form, their response is automatically recorded in your excel workbook.
