How to apply to test with State of Florida?
Step 1: Go to http://www.floridahealth.gov/licensing-and-regulation/emt-paramedics/index.html
Step 2: Scroll down and to the bottom right of the screen click on Apply for a License.
Step 3: Here you will Create an Account. (click create an account)
Step 4: Click either Standard Application or for my Veterans, Click Active Military or Veteran Application (VALOR)
Step 5: You will be prompted to create a log in and password. You will then need to pick from the drop down (Board of Council — Bureau of Emergency Medical Services) & (Profession — Emergency Medical Technician or Paramedic)
Step 6: Enter in all of your personal information, i.e.; the school in which you completed either EMT or Paramedic, EMT’s your BLS/CPR card information will need to be entered or if you are applying to test as a Paramedic you will need your ACLS card information (make sure these cards are current)
Step 7: Pay the application fee via any major credit card.
EMT’s $35.00
Paramedic’s $45.00 online and $40.00 to the testing center on the day of the test. (Unless you are veteran, this fee is waived)
Step 8: You are done, print your receipt and make sure you send the State of Florida, Bureau of EMS a copy of your course completion (EMT or Paramedic) and a copy of your BLS/CPR card or ACLS Card.
