INSTRUCTIONS TO FILL OUT ABSENTEE BALLOT:
The absentee ballot comes with 3 envelopes
1.One plain (the secrecy envelope);
2.One with an affidavit, or oath, printed on the outside;
​3.One plain pre-addressed envelope, (the outer envelope)

[Check the box “I am physically incapacitated and will not be able to vote in person on election day.” if you are voting absentee due to COVID-19 concerns.]
Once you vote the ballot, the procedure is as follows:
1. Seal the ballot in the plain envelope
2. Place the plain envelope inside the accompanying affidavit envelope
​3. Seal affidavit envelope & complete the affidavit on the outside of the envelope
4. For your vote to be counted, you must sign the affidavit witnessed by either a notary public or 2 witnesses 18+ years old and have them sign too. (Unless 65+ or underlying medical condition — see details above)
5. Write your address on the outer envelope.

IF -required by an employer under unforeseen circumstances to be unavailable at the polls election day

MEDICAL EMERGENCY ABSENTEE VOTING
Applications can be made by a voter who has a medical emergency requiring treatment from a licensed physician within 5 days of an election. During that 5 day period, the medical emergency absentee ballot application AND the voted absentee ballot must be returned no later than 12pm on the day the of the Election.
The medical emergency absentee ballot application requires that the attending physician describe and certify the circumstances as constituting an emergency. ​

How to Vote Your By-Mail Ballot
In your voting packet you will receive a ballot, a return mailing envelope that contains the voter certificate and ID requirements, instructions and a gray secrecy sleeve.
Vote your ballot by fully filling in the ovals next to your choices in blue or black pen.
Place voted ballot inside gray secrecy sleeve. Place gray sleeve containing ballot inside return mailing envelope.
In the presence of a witness, sign your ballot envelope and provide at least 1 ID.
A witness must be someone 18+ years or an authorized official (Notary Public, municipal clerk, or any other official authorized and willing to administer the oath).
Secure ballot by folding over the flap and sealing the flap at the top & bottom.
Apply correct amount of postage amount as stated in the instructions.
Mail your ballot. It must be postmarked on or before Election Day.

How to Vote Your By-Fax Ballot
You will be faxed a ballot, a Voter Certificate and Identification sheet and instructions.
Vote your ballot by filling in the ovals next to your choices.
In the presence of your witness, sign your Voter Certificate and Identification sheet and provide at least ONE identifier.

When returning the ballot by fax, you will be provided with instructions and a return fax number to return your voted ballot and Voter Certificate and Identification sheet. Voter Certificate, ID sheet and ballot must be received on or before 8PM on Election Day.
How to Vote Your Online Delivery Ballot
You will be emailed a notification of ballot availability and instructions. Open the link to the website that will be provided in your email and follow the instructions provided in the email. In addition, the system instructions will guide you through the online system.
Log into the online delivery system using your information.
Vote your ballot by selecting your choices on your computer screen.
At the end of voting, you will be prompted by your computer to download your voted ballot. After opening, you will print your ballot.
Next, you will choose the method that you wish to return your ballot. Upon selecting, you will be prompted to download a PDF file of your Voter Certificate and Identification sheet and other voting materials to be used to return your voted ballot that you will print.
In the presence of your witness, sign your Voter Certificate and Identification sheet and provide at least ONE identifier.

How to vote an early ballot?
Step 1. Request your early ballot (either one time request or sign up for PEVL).
Step 2. Retrieve your ballot from your mailbox.
Step 3. Vote your ballot.

Here’s what you’ll find inside:

• Ballot — one page, front and back
• Ballot only envelope

Once you’ve filled out the ballot, you’ll seal it by itself in the included “ballot only” envelope. The voter statement and copy of your ID goes in the outer return envelope along with the “ballot only” envelope.
Putting your voter statement in with the ballot in the “ballot only” envelope will result in the ballot becoming provisional and possibly not being counted.
TAKE YOUR TIME — If there’s any incorrect or missing information, your votes WILL NOT COUNT. You will NOT get the chance to fix this later.
You can drop it off at your county clerk’s office or mail it back in that included envelope. If you mail it back, you should use two stamps. Either way, your ballot must be returned to the county clerk’s office by the end of the day Tuesday, November 3.

The voter expects to be confined, due to illness or injury, or the voter expects to be caring for a confined person at a private residence, on Election Day.
The voter is a voter with disabilities and believes their polling place is not accessible to them.

EMERGENCY BALLOT

Drop Boxes
One ballot drop box is available outside of each Ohio county’s board of election offices, per an order from Secretary of State Frank LaRose that banned counties from offering more than one. An Ohio judge said restricting the number of boxes is “arbitrary and unreasonable,” but the judge didn’t bar LaRose from enforcing the order.
LaRose’s office later said the order remains in effect, and it plans to appeal the judge’s opinion.

While filling out your application
Where you see “Select the elections you are requesting an absentee ballot for” check “All” in order to receive ballots for every upcoming election you are able to vote in.
Once you’re done filling out your absentee ballot request form, have someone look it over if you’re able.
If you have an immunodeficiency disorder or a circumstance which hinders you from leaving your home to get your ballot or vote in person, you may fill out the bottom section titled “Authorized messenger request due to sickness or disability only” to establish your authorized messenger.
Be sure that every required field is filled in before signing the form at the bottom and sealing it in your envelope to be mailed in. If you don’t want to lick your envelope, consider taping it shut or dampening a cloth and running it along the tab where the seal is.

After a voter receives and fills in their absentee ballot, they need to sign the back of the manila envelope with the same signature they used on their ballot application

Minnehaha County — Can take ballots to one of 2 drop boxes located on county campus in downtown Sioux Falls by 7PM on Election Day. The drop boxes are located outside the main entrance of the county administration building and along the stairs on the north end of the election center.
Lincoln County — Plans to place a drop box in parking lot of Faith Baptist Church at 57th Street & Minnesota Avenue in Sioux Falls every Wednesday 3 to 7PM starting Sept. 30. The drop box will be guarded by a Lincoln County Deputy and returned to the auditor’s office the same day.
Can also hand deliver completed absentee ballot to County Elections Official on Election Day with enough time for them to deliver the ballot to your precinct

If you check “Disabled” — DO NOT WRITE IN YOUR DISABILITY.
Avoid any confusion by election officials and sign both your application and ballot in the same way.
Have a ballot mailed to the address where you actually are.

EMERGENCY If a voter is a sequestered juror or hospitalized, then the voter can apply for an absentee ballot by 5pm on Election Day.
You need to certify on the application that you are “indefinitely confined,” meaning that for reason of age, illness, infirmity or disability you are requesting a ballot for all subsequent elections. Once you are on the permanent absentee list you will stay there until you are no longer confined or you fail to return a ballot.
