Hospital Acquired Infection (HAI) or Nosocomial Infections is defined as infection that was neither present nor incubating at time of admission of patient. This includes infections appearing after discharge and also occupational infections among the staff of the facility.

Common HAI’s include Urinary Tract Infection (UTI), Surgical Site Infection (SSI), Ventilator Associated Pneumonia (VAP), Blood Stream Infection (BSI).

HAI’s are acquired by patients notoriously in Operating theatres, ICU’s, and wards within the hospital set up. They are caused by bacteria, virus and fungi.

Majority of cases are caused by Acinetobacter, Aspergillus niger, Bacillus cereus, Bacillus subtilis var. niger, Enterobacter, Escherichia coli, Klebsiella, Micrococcus luteus, Pseudomonas aeruginosa, Saccharomyces cerevisiae, Serratia marcescens, Staphylococcus aureus, Streptomyces, Candidas albicans.

Sources of infection

  • Endogenous origin - organisms present normally in patient’s flora
  • Exogenous origins include Airborne transmissions, Unhygienic practices of medical professionals, Drains and catheters

Burden

It affects both patients as well as the hospital: Economic burden for patients, Additional workload on hospital facilities and staff.

The effects of HAI on patient includes functional disability, emotional and economic burden and in some cases, death.

It is vital that hospital authorities, in both government and private sector, focus more on infection control. With few or rare peer reviewed publications, the magnitude of HAI in India is not known as of yet. Publications by WHO, INICC peg the average incidence rates at 5–10% and up to 28% in ICU.

Nosocomial control programmes in hospitals are at nascent stage in India and some are yet to establish Central Sterilization and Supply Department (CSSD).
