The original article seemed a glorified description of how incompetent management blame their subordinates for failures, are totally uninterested in finding the root cause, and instead of fixing what’s broken, they just blame the guy who’s easy to blame, rewrite the whole f**king thing and wash their hand off it.

They might even write an article or two about the process.
