Filling out the live scan form can be confusing, especially when you are new to licensing. In this article, I will cover all the steps you need to take to fill the Live Scan form.

What is a Live Scan form?

Live scan involves electronically submitting digital fingerprints to the California Department of Justice (DOJ). This automated service facilitates criminal history background checks, often mandated for employment, licensing, certification, foreign adoptions, or VISA/Immigration clearances.

You need to look at the form in four parts.

Part 1: Get a pre-filled form from California Department of Insurance website.

The first two sections of the form as indicated in the image above should be prefilled before you go for your fingerprinting appointment.

To obtain a pre-filled form for insurance producer license, follow these steps:

  1. Go to California Department of Insurance website.
  2. In the search bar, type 8016.

3. Tap search.

4. Click the first link that appears titled BCIA 8016 — Request for Live Scan Service.

5. Download the form.

Part 2: Fill out your personal information

This section is pretty straightforward. Add all of your personal information here.

Part 3: More Pre-filled Info

This section is also pre-filled and you don’t need to take any action here.

Part 4: Fingerprinting company’s responsibility

The section in blue is filled by the company you are getting fingerprints done at.

Once done, take your form to your fingerprinting appointment.
