One sneaky anti-pattern that can creep in is when the leader confuses (vision, goals) with (prescriptive plan, vanity metrics, and goals-per-person rather than the company).

I’m positive you aren’t guilty of that, but it happens with leaders who really believe they are in sync with the approach you described.
