Hello Sinha,
From my knowledge:

1. In my opinion, yes. When it comes to hiring a foreigner, the German government would check if employers pay reasonable salary, the basic rules is the payment can not lower than minimum wage, which is around €1,498 per month /year of 2018.
If you are interested in average salary in Germany, please check PayScale or GlassDoor.

2. ZAV pre-approval and working visa are reviewing by different official departments. Some required documents are the same, but applicants should submit two piles of required documents, one for ZAV pre-approval (issued by Federal Employment Agency), another pile for working visa (issued by German Embassy).

FYI: the normal flow is applying ZAV pre-approval in advance, and then includes ZAV approval letter in your visa application documents.
