Here is a problem statement from one of the major hospital.

• On an annual basis, Accountable Care Organizations (ACOs) are required to accurately report “Key Metrics” that is used to assess quality performance. This is necessary in order for the ACO to be eligible to share in any savings generated.
• Improve Outcomes of every specialty divisions of the hospital
• Without access to transparent, actionable data, leadership was unsure if improvements would be sustained, let alone if existing workflows could lead to new improvements
• Hospital, is comprised of seven hospitals, 750 employed/aligned providers, and one of the largest Medicare Shared Savings ACOs in the nation. They had been using the LACE index to predict risk for readmission, and while it was helpful, Hospital’s patient population was different than the population used to develop the LACE index, leaving the health system with some uncertainty regarding the readmission risk of its patients
