Details of incident

An employee’s right eye was injured by a spark while welding on the production line. Fortunately he was taken to the hospital by a co-worker within 20 minutes. The employee, Finn Chen, is currently recovering at the hospital, and the doctor has mentioned that it will take him two months to recover from this serious injury.

Assessment

While Mr. Chen was welding on the production line, his protective mask suddenly dropped. After investigating the situation, it was found that Mr. Chen followed all the safety procedures appropriately and had worn the required protective gear. It was discovered that the incident was caused by the quality of the Velcro fastening.

Action taken

I have asked the Equipment Maintenance Manager to inspect all the protective gear within a week and make a list of the condition of all the gear and tools used on the production line. Furthermore, all gear and tools will be checked regularly in the future. I also visited Mr. Chen at the hospital and guaranteed him that he will be well-compensated for what had transpired and that his hospital expenses will be completely taken care of by the company.
