Incident Report (in class practice)

Details of incident:

An electrician who was working at our factory got an electric shock and fainted while testing a high voltage product. Fortunately, it happened at a time of day when there were a lot of colleagues on site who immediately turned off the power and then called me on the internal emergency telephone. I called for an ambulance that arrived about 10 minutes later. The employee Brandon Bear is currently recovering at the hospital where the ambulance took him. The doctor has reported that the electrician has only a few burns and is recovering well.

Assessment:

Mr. Bear was at the Quality Control Department disassembling a refrigerator and conducting some regular electric checks. He touched a component of the refrigerator that electrified him because he was not wearing the safety gloves provided. He accepts that the injury is his fault.

Action taken:

I have asked employees working in our factory to follow the stipulated rules and have put up more warning signs at conspicuous positions. I have also visited Mr. Bear at the hospital and conveyed best wishes on a quick recovery on behalf of the company.
