As a submarine officer, I was responsible for approving and supervising boat maintenance. As you can imagine, the mechanical and electrical systems onboard this $420 million dollar machine were complicated and complex. It was critical- literally a matter of life or death- that maintenance was done correctly, according to process and procedure. To achieve this, the Navy requires that the area receiving maintenance be isolated from the rest of the system by shutting off not one, but two breakers or valves that connect them. Then a tag was hung to let everyone know not to touch or use them. Tags were checked by two independent people to ensure the system was isolated correctly. As I’ve already described, it was imperative this was done correctly for the safety of the ship and crew.
