Clover Partners with the American Academy of Home Care Medicine to Advance House Calls

At Clover, we’re dedicated to improving the quality of life of our most vulnerable members and have taken a personalized, proactive approach to improving their health. As a geriatrician, I’ve seen firsthand the impact house calls programs can have on frail and at-risk individuals, and that’s why we’ve focused on building out a robust and technology-enabled program that helps our providers deliver exceptional care.

Our In-Home Care program takes the benefits of concierge-style medicine — better access to care and additional care resources supported by machine learning technology — without excluding people that can’t afford it. For older adults, many of whom are on a fixed income, this is especially important. Not only is care in the home more convenient and comfortable for many patients, it is often the only option for the patients who find getting to a doctor’s office physically impossible. House calls can also significantly lower healthcare costs for high-risk adults when compared to traditional office-based practice. Unsurprisingly, deep engagement in the home can also reduce adverse health outcomes including hospital readmissions.

At Clover we’ve taken a technology-enhanced approach to house calls: both in how health plan members are identified for the program, and the tools available to the clinical practice to improve member health outcomes. We use data analytics and machine learning to evaluate which members are at risk of adverse health behaviors and outcomes (for example, those inconsistently adhering to medications or frequently going to the ER for avoidable reasons), and then pinpoint the individuals that would most benefit from primary care delivered in the home. We arm our providers with comprehensive data rarely available at the point of care and further support their efforts with contributions by pharmacists, nurses, and social workers. Clover’s ‘behind the scenes’ technology allows our clinicians to focus on the patient and tailoring care to their needs, as opposed to digging through charts searching for information.

As a demonstration of our continued commitment to house calls, we’ve recently joined the Industry Relations Council of the American Academy of Home Care Medicine (AAHCM), the leading professional organization and advocacy group for the industry.

AAHCM is responsible for creating some of the most important resources for clinicians providing home-based care including: developing the first ever Ethics Statement for the field, developing and conducting the first medical director training seminars for home health agency medical directors, administering and developing the Home Care Certification Exam, representing the interests of members in a myriad of regulatory issues from audits to quality and incentive reporting programs, and much more.
