Empowering Doctors to Deliver Exceptional In-Home Primary Care (Clover is Hiring!)

Our healthcare system wasn’t built to care for our most vulnerable patients, many of whom have multiple chronic health conditions, advanced illnesses, and complex social histories. Rather than receiving care that is accessible, comprehensive, and integrated across providers and care settings, many of these patients experience rushed outpatient visits, a constant rotation of providers, and serious difficulty leaving the home to access necessary services.

As a practicing geriatrician, cardiologist, and Chief Scientific Officer at Clover Health, I have seen these problems all too often. Yet there are great opportunities to improve the lives of older adults.

At Clover, we are committed to caring for our most complex members through our in-home primary care practice. We’ve taken a personalized, proactive approach to health that puts patients first, and we’re doing it differently than traditional insurers. With technology as our backbone, we use data analytics and machine learning to evaluate which members are at risk of adverse health outcomes (for example, those missing doctors visits or inconsistently adhering to medications). We can then pinpoint high-risk members that would most benefit from in-home care.

Our in-home care program takes the benefits of concierge-style medicine — more personalized, proactive care supported by machine learning technology — without excluding people that can’t afford it. For older adults, many of whom are on a fixed income, this is especially important. Not only is in-home care often more comfortable, getting to a doctor’s office can be physically impossible for millions of homebound and home-limited seniors. In-home care can also significantly lower healthcare costs for high-risk adults, when compared to traditional office-based practice. Unsurprisingly, deep engagement in the home can also reduce adverse health outcomes including hospital readmissions.

Given these benefits of in-home primary care, I’m excited to share we are expanding Clover’s team of in-home primary care physicians. Our doctors are at the forefront of truly transforming care for older adults, using Clover’s technology to make personalized and impactful visits to seniors’ homes.

As a doctor at Clover, you are supported by a nurse care coordinator, medical assistant, social worker, and pharmacist. Additionally, you don’t need to worry about whether someone is insured or what is covered, because all your patients are Clover members. The health plan exists to drive your effectiveness, not block it.
  • Focus on value, not volume: Clover’s goals are to improve our members’ quality of life and experience with the healthcare system, while also reducing adverse health outcomes.
