Place-based Health Conference: Agenda & Update
Crisis in NHS and social care services is the clearest sign yet that we need a new model of health, care and support.
In 2016 Collaborate and the NLGN launched Get Well Soon — the final report of the Place Based Health Commission, chaired by Lord Victor Adebowale. The Commission argued that this new model must be place-based and fundamentally collaborative. At our conference in partnership with NLGN on 2 February, we will be asking system leaders and stakeholders what it will take to turn this aspiration into reality. It will be a vital input for those working on Sustainability and Transformation Plans (STPs) and health and social care integration within local settings.
