[eBook] The Pediatrician’s Hummingbird: Exploring Injury Profiles, Navigating & Measuring Patient Journey for Pediatric Concussion Management in Youth Football

--

This book consists of two parts. The first part describes the stages of adolescence and emerging adulthood, and the effect of trauma on the developing brain. General information on adolescent behavior, incidence and recurrence of concussions, trauma related to soccer injuries, academic performance and outcomes, development outcomes, the role of the family is covered. The role of pediatrician oversight in concussion resolution, brain injury criteria, and injury profiles are discussed in detail in the first section.

In the second section on navigating and measuring the sports related concussion (SRC) journey, pediatric trauma management, trauma-informed care, AAP policy, and the multidisciplinary nature of pediatric SRC management is discussed. The major part of this section covers concussion clinical care guidelines update, from the Children’s Hospital of Eastern Ontario, 360 Concussion Care, Brain Injury Foundation, CDC mTBI diagnosis and management guidelines, and CDC mTBI management guidance for sports medicine providers. Research studies on pediatric patient flow in trauma care is explained, which was executed with a process mining approach developed in a level I trauma center. This led to the identification of key patient pathways and care transitions.

Resources on emergency management protocols, severe TBI management, ophthalmologist guidelines for pediatric trauma, and ICU nutritionist guidelines are identified. Research-based guidelines on pediatric trauma care in low resource settings is explained, which includes pre-hospital guidelines, systems triage, emergency management, surgical management, LMIC-specific guidelines, and specific injury management protocols.

Further, rehabilitation protocols covering communication and swallowing protocols, physical therapy guidelines, CDC protocol for neuropsychologists, guidelines for physiotherapist, chiropractor, occupational therapist, otolaryngologist, radiologist, audiologist, optometrist, recreation therapist, and social worker are included. Guidelines for making referrals, and return-to-sport and return-to-learn protocols are identified.

Furthermore, concussion service delivery models (SDMs) and cost analysis are presented and social determinants of health in diagnosing, and managing concussions are presented. Evidence on pediatric TBI outcomes is presented including health-related quality of life (HRQoL) following concussion, persistent post concussion symptoms (PPCS), effect of SRC on cognitive function and concomitant ADHD, physical activity after concussion, rest-based concussion management, mental health outcomes, altered brain structure and function, sleep quality, headaches, visio-vestibular and ocular outcomes, academic outcomes, parental experiences following pediatric concussion, behavioral and emotional outcomes, and health services utilization are discussed. Finally, outcome measures, treatment adherence, and concussion quality measurement is explained.
