When I was researching information for my article about costly bugs, I discovered that plenty of them are related to NASA. The teams were building big and complex systems. A lot of engineers were involved. With such talents, it should be possible to avoid disasters.

Yet, disasters happened and caused the loss of human lives.

Looking deeper, we will see big mistakes in leadership styles and culture. Nowadays, all leaders of successful companies stopped doing that. They learned from the mistakes of others.

So what are the lessons anyway?

Let People Speak Freely

We assume that strong leaders speak confidently and act bold, they know what they are doing. Their surroundings trust them and do not question anything they do. Because if the person was capable of achieving a high position they surely have full credit.

Unless they fail and reality hits them hard. They are not leaders, they are bosses.

True leaders remain close to their teams and are always open to hearing questions and hesitation from their teammates. Everybody is not afraid to share their concerns and will be heard by the management.
