An Alcohol and Drug Evaluation Uniform report is a very specific assessment that is conducted for individuals arrested in the State of Illinois for driving under the influence of mind-altering substances. The individual should always request to observe the provider’s DUI Evaluation license that is issued by the State of Illinois.
