Shiro Ishii was commissioned as a military surgeon in 1921 by the Imperial Japanese Army. As a distinguished doctor in the field of microbiology he received the following promotions throughout his career: 1931 — Senior Army Surgeon, Third Class; 1935 — Senior Army Surgeon, Second Class; 1936 — Director Unit 731; 1938 — Senior Army Surgeon, First Class; 1941 — Assistant Surgeon General; 1945 — Surgeon General.
