Among its other profound effects, the COVID pandemic has accelerated interest among healthcare professionals in both the issue of health equity and the opportunity for population health management. However, questions remain about the efficacy and economic viability of population health management. How do you define “success” in population health management? What does successful implementation look like? How do you measure effectiveness? This session will explore these issues as they pertain to a particularly large and vulnerable population that is commonly overlooked: veterans, service members, and their families (aka the Warrior Community).

Join us to explore:

  • Implementing a cost-effective population health management model
  • Integrating population health management and cultural competence to enhance ROI
  • Using the Warrior Community as a “best practice opportunity” for population health management efforts
  • How outreach and communication increase program effectiveness and ROI
